What the evidence shows about how long teeth whitening lasts
How long does teeth whitening last depends entirely on the chemistry behind the method. Charcoal vs peroxide whitening: mechanism, longevity, and safety
Charcoal’s popularity rests on the assumption that black powder naturally lifts stains. The mechanism is entirely physical (scrubbing), not chemical. When tested in a controlled trial, charcoal whitening showed no color improvement over baseline. 2 Peroxide methods, by contrast, alter stain molecules through oxidation, a chemical change that persists until new extrinsic stains accumulate or intrinsic color returns. 9
You’re not choosing between two whitening methods with different durations. You’re choosing between one method that works and one that doesn’t.
Why charcoal doesn’t whiten (and what it actually does)
Charcoal toothpastes and powders work by abrasion. The activated charcoal particles physically scrub the tooth surface, removing some superficial staining. That sounds plausible until you look at the evidence.
Ribeiro and colleagues randomized participants to charcoal whitening toothpaste or control (standard dentifrice) for four weeks. At the end of the trial, there was no statistically significant difference in tooth color between groups. 2 The charcoal group did not whiten. The scrubbing removed surface debris, but it didn’t change the underlying color of enamel or dentin.
A 2022 meta-analysis on charcoal-based whitening products confirmed the mechanism’s limits. The studies reviewed showed that charcoal products increase surface roughness and reduce enamel microhardness, consistent with abrasion. 8 Higher abrasion values do remove stains, but they also wear enamel. Once enamel thins, the yellow dentin underneath becomes more visible, paradoxically making teeth look darker over time.
The marketed claim is “natural whitening.” The clinical reality is no whitening, with potential enamel harm.

How peroxide whitening works (and why results last)
Peroxide whitening uses hydrogen peroxide or carbamide peroxide (which breaks down into hydrogen peroxide and urea). The hydrogen peroxide molecule penetrates enamel and dentin, breaking apart chromophore molecules, the organic compounds responsible for tooth discoloration. 9
The oxidation process cleaves carbon double bonds in these chromophores, turning them into smaller, colorless molecules. This is a chemical transformation, not a physical removal. The stain molecules themselves change structure. 9 That’s why results persist. Once a chromophore is oxidized, it doesn’t spontaneously reassemble.
What eventually returns is new staining. Extrinsic stains from coffee, tea, red wine, and tobacco accumulate on the enamel surface. Intrinsic discoloration from aging or certain medications slowly darkens dentin over time. The original peroxide-induced whitening fades not because the chemistry reverses, but because new pigment builds up. 1
How long that takes varies by method (in-office vs at-home) and by individual behavior (diet, oral hygiene, smoking). The chemistry itself is stable. The environment is not.
How long bleached teeth last: professional vs at-home methods
The longest follow-up in the published literature is 18 months. Auschill and colleagues tracked participants who completed a home bleaching regimen with 6% hydrogen peroxide. At the 18-month mark, tooth color remained significantly lighter than baseline, though some relapse had occurred. 11 The majority of the whitening effect persisted beyond a year and a half.
At the one-year mark, a randomized trial of 10% carbamide peroxide (equivalent to roughly 3.5% hydrogen peroxide) found that at-home bleaching maintained visible color improvement over baseline. 10 Participants did not use maintenance trays during follow-up, yet results held.
In-office bleaching typically uses higher concentrations (25% to 40% hydrogen peroxide) with shorter application times. A split-mouth study comparing in-office and at-home methods found that both achieved similar color improvement, and both maintained results at six-month follow-up. 5 The higher concentration did not translate to longer-lasting results. It translated to faster results.

Four factors determine how quickly whitened teeth return toward baseline color.
Extrinsic staining from diet and habits. Coffee, tea, red wine, berries, soy sauce, and tobacco all deposit surface stains. These are the first to return after bleaching. 1 Individuals who consume high-tannin beverages daily will see faster relapse than those who don’t. The peroxide-induced whitening is still present underneath, but the surface layer darkens.
Intrinsic discoloration from aging. Dentin naturally darkens with age as secondary dentin forms and enamel thins. Peroxide can lighten existing intrinsic stains, but it doesn’t halt the biological process that generates them. Over time, intrinsic yellowing resumes. 1
Enamel porosity and thickness. Thinner enamel allows more dentin color to show through. Individuals with naturally thin enamel or enamel worn by erosion or attrition may see faster relapse because the underlying dentin color dominates. Peroxide penetrates enamel to reach dentin, but thinner enamel also means less masking of dentin hue. 9
Maintenance bleaching frequency. Some users apply maintenance trays every few months to prevent relapse. The literature on maintenance protocols is thin, but one long-term study noted that participants who applied occasional touch-up bleaching maintained whiter teeth at 18 months compared to those who didn’t. 11 Maintenance isn’t necessary for results to last 6 to 12 months, but it extends longevity beyond that window.
How long can teeth whitening last for someone who avoids staining foods, doesn’t smoke, and uses maintenance trays? The 18-month data suggest at least that long, with gradual, manageable relapse. 11 For someone who drinks coffee twice daily and never uses maintenance, 6 to 9 months is more realistic. 1
If you’re evaluating whitening longevity, focus on peroxide concentration, application duration, and your own staining habits. In-office versus at-home matters less than whether you drink coffee daily and whether you plan to use maintenance trays. The chemistry works the same. The environment around your teeth determines how long it shows.
Side effects: charcoal vs peroxide
A 2026 meta-analysis on enamel surface changes from whitening procedures reviewed 47 studies. The conclusion: peroxide-based whitening at concentrations used in clinical trials (10% to 40% hydrogen peroxide, 10% to 22% carbamide peroxide) produced no clinically significant enamel damage. Surface roughness and microhardness changes were minimal and reversible. 3
Transient tooth sensitivity is the most common side effect of peroxide whitening, reported by 30% to 60% of users. The sensitivity typically resolves within 48 hours after treatment ends. 6 It’s caused by temporary inflammation of the dental pulp as peroxide penetrates enamel and reaches dentin tubules. Lowering concentration or shortening application time reduces sensitivity without eliminating efficacy. 12
Charcoal, by contrast, damages enamel through abrasion. The 2022 meta-analysis on charcoal whitening products found that charcoal toothpastes significantly increased surface roughness and decreased enamel microhardness compared to standard dentifrices. 8 Rougher enamel traps more plaque and stains more easily. Lower microhardness means the enamel is more vulnerable to wear.
The irony: charcoal is marketed as a “natural” and “gentle” alternative to peroxide. The evidence shows the opposite. Peroxide at studied concentrations is safe for enamel. 3 Charcoal abrades it. 8
